Click the Benefit Plan sub-tab.

Click Edit to display the Benefit Plan window.

Update the plan information as follows

Plan Received

Check Yes if the plan has been received. Otherwise check No.

Donor Coverage

Check Yes if the plan includes donor coverage. Otherwise check No.

Lifetime Maximum

Lifetime maximum benefit under the plan goes here.

Annual Maximum

Annual plan maximum amount goes here.

Transplant Rider

Check Yes if the benefit plan includes a transplant rider. Otherwise check No.

Effective Date

When the transplant rider became effective. Enter the date directly in the field in MM/DD/YYYY format or click the calendar icon and set the date using the date picker tool.

Medicare Effective Date

When Medicare coverage became effective. Enter the date directly in the field in MM/DD/YYYY format or click the calendar icon and set the date using the date picker tool.

Date Medicare Primary

Medicare primary date. Enter the date directly in the field in MM/DD/YYYY format or click the calendar icon and set the date using the date picker tool.
